Direct Care Services

Supported Independent Living (SIL)

Ndiho will also be offering Supported Independent Living (SIL) solutions to our participants. This tailored level of high-needs care is designed to help individuals sustain a functional home environment, develop skills, and maintain healthy relationships within their immediate community. We will help our clients to ensure we offer suitable SIL packages with detailed supports that suit their specific circumstances, care needs and living requirements to organise the absolute best care. Support Coordination

Coordinating support begins with gaining a deep understanding of what an individual’s needs, goals and current circumstances are. We use this information to create tailored support plans in alignment with your funding to help you actively participate in areas of interest, while introducing skills that elevate confidence and independence.

This includes:

  • Collaborating to comprehend your strategy and financial resources
  • Establishing appropriate assistance
  • Developing your ability to be self-sufficient
  • Acquiring customised services that match your requirements and objectives
  • Exercising your autonomy and authority

This will be delivered by our highly experienced team of Occupational Therapist and social workers. The tailored plan of each individual will then be implemented by support workers with experience in specialist positive behaviour support.

Specialist Support Coordination

This represents an elevated level of support coordination. A Specialist Support Coordinator will perform all the tasks mentioned in Coordination of Supports, and additionally aid you in addressing difficulties in your support system while ensuring reliable service provision. It is designed for individuals with intricate circumstances requiring specialised assistance. Our services can assist you in organising your resources to optimise the benefits of your NDIS plan.

This includes:

  • Helping participants reduce complexity in their support environment, so they can connect with funded supports as well as broader supports
  • Negotiating support solutions with multiple stakeholders to achieve well-coordinated plan implementation
  • Help manage crisis points for participants, ensuring participants have access to relevant supports during a crisis.

This will be delivered by our highly experienced team of Occupational Therapist and social workers. The tailored plan of each individual will then be implemented by support workers with experience in specialist positive behaviour support.

Specialist Positive Behaviour Support

Behavior support involves devising personalised plans for individuals with disabilities that cater to their requirements, while minimising or eliminating the use of regulated restrictive measures.

This includes:

  • Improve people’s wellbeing and quality of life
  • Reduce people’s behaviours of concern
  • Reduce and eliminate restrictive practices
  • Help practitioners understand the best ways to provide positive behaviour support
  • Increase the number of skilled and capable practitioners working in the sector

This will be delivered by our highly experienced team of qualified and highly experienced DEs, Occupational Therapists and social workers (or other) who are NDIS Registered Behavioural Practitioners and who have great outcome history – ie who have strong experience in achieving successful PBS outcomes. The tailored plan of each individual will then be implemented by qualified therapists and support workers with experience in specialist positive behaviour support.

Early Childhood Supports

Our team of Allied Health professionals with experience and expertise in working with children will provide assistance to all children aged 0-6 years with developmental issues or disabilities and their families, to attain improved long- term results, irrespective of the diagnosis. Additionally, we offer the development and implementation of personalised behavioural support management plans.

The team will work with you to understand your child’s needs and tailor services that best meet your child’s needs.

Various Therapies

Our highly experienced team of allied health professionals deliver therapeutic support to participants including Occupational Therapists and Social Workers. We also provide evidence, assessments and reports to inform NDIA liaisons including AT, funding levels and access.
